Support Coordination and Specialist Support Coordination

Paramount Care SA works closely with you and your family or support people to understand you and your needs, and provide you with high quality support coordination. The support coordination services are person-centred and individually tailored.

We are committed to work with you to get the best use of the supports from your NDIS plans. We are here to listen to what you want, find out what works best for you all within the framework of the NDIS and connect you with the best support team. We walk through this NDIS journey with you.

There are three levels of support coordination that can be included in your plan:

  1. Support connection – builds your ability to connect with informal, community and funded supports so that you can get the most out of your plan and pursue your goals.
  2. Coordination of supports – builds the skills you need to understand and use your plan. A support coordinator works with you to make sure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be part of your community.
  3. Specialist support coordination – a higher level of support coordination for people with more complex needs and situations who need specialist support. A specialist Support Coordinator will assist you to manage changes and help ensure consistency.

Community Participation and Transport

As part of core support in NDIS, community participation enables you to engage with different social and recreational activities in the community. By emphasizing choice, control, and personalized support, we create exceptional opportunities for individuals with diverse needs to participate in social and community activities.

Our model is designed to foster inclusivity and empower our participants to lead more fulfilling lives and actively participate in social activities at their wish. This program recognizes the unique needs and aspirations of each participant, tailoring support to help them achieve their goals.

We also aid with transport to meet any commitments you may have. These can be housing appointments, medical appointments, participation in community events, grocery shopping or a leisure trip to destinations of your choice. This support provides access to disabled-friendly vehicles, travel assistance and learning the skills to use public transport.

You will be supported to actively participate in any community activity that you wish such as

Exploring local shops, libraries and recreational places

Attending appointments

Attending social and cultural programs

Travelling to visit friends and family

Participating in group-based activities like sports

Socialising with people in the community

Going to excursions with your group

Participating in activities such as drama, acting, singing, etc.

Taking part in interactive activities

Searching for a job and applying to the same

Developing the skills required to pass job interviews

Assistance with daily living

Paramount Care SA provides this service to support you in your daily activities so that you can live as independently as possible in your home or community. If you can look after yourself but need some extra help, our Assistance with Daily Living services can make it possible. A qualified and friendly team member will visit you in your home for several hours each week, depending on how much support you need.

Personal care: Assistance with activities like showering, dressing, and grooming.

Meal preparation: Planning, preparing, grocery shopping and cooking meals

Household tasks: Cleaning, laundry, dishwashing, and other routine household chores

Gardening

Medication management: Managing and administering medications as prescribed by a healthcare professional

Home maintenance: Minor repairs and maintenance jobs to ensure the home environment is safe and accessible.

Mobility: Moving around the home or community, which might include transferring from a bed to a wheelchair or using mobility aids

Staying active: Engaging in health and fitness activities

Assisting with developing individual skills and completing goals

Community Nursing

Our caring nursing professionals are highly skilled and up to date with current knowledge and practices. They are also culturally sensitive, ready to help and are there to support you to meet your clinical and personal needs so that you can lead an independent and healthy lifestyle.

We work in collaboration and consultation with your health practitioners like your GP, specialist, and others involved in your care. Our nurse helps you get on the road to recovery as quickly as possible. Our in-home community nursing services are offered in the comfort of your own home following an illness, hospital stay or medical procedure to ensure quick recovery.

Before commencing, a registered nurse will assess your clinical need for nursing and/or personal care services. The registered nurse will then determine the type and frequency of nursing and personal care services you need in consultation with you.

Some of the services offered by the community nurses include:

Developing treatment plans

Case management

Medication administration/supervision

Visiting nurse and assessment

Wound care

Dementia management

Overnight nursing care

Education for participant and their families

Community Nursing

Joining group activities including sports encourages social connections and promotes personal growth and enjoyment. This is an excellent way to improve your physical, mental, and emotional well-being.

At Paramount Care SA, we assist you to participate in recreational and social activities in the community. Our lovely team will assist you enjoy a wide range of activities and programs aimed at ensuring you live a happy and fulfilled life.

Respite Home Care

Respite care is there to support you and your carer for short periods of time. Home care is essentially crucial in facilitating the process of recovering from an illness or injury and transitioning out of hospital.

Paramount Care SA offers home care supports which gives your loved one the critical assistance they require and gives you the peace of mind you need.

We will arrange the respite care you need when you need it. Our qualified staff provide specialised care are there to walk with you throughout your journey, whether it’s weekdays, weekends, evenings, overnight stays or 24-hour care.
